About the Role

The Dialysis Liaison is an integral member of the Nephrology Program, responsible for organizing and providing home haemodialysis treatment in the community. This role considers individual patient needs, time, supplies, and equipment requirements in consultation with a registered nurse.

Responsibilities

• Assist home hemodialysis patients with treatments in their homes according to the treatment plan, promoting quality of life.
• Complete and maintain related records and documentation.
• Continuously observe patients, assist with necessary machine alarms, and report status changes to the primary registered nurse.
• Assist the team with necessary supplies for haemodialysis treatments.
• Ensure correct use of equipment according to guidelines and protocols, and report malfunctioning equipment.
• Follow all infection control precautions and protocols under the direction of the Registered Nurse.
• Report patient and family requests or complaints to the Registered Nurse.
• Report any other concerns or issues to the Registered Nurse as required.
• Work safely and ensure the safety of others by following established procedures and policies.
• Maintain confidentiality in all aspects of work.
• Attend and complete all required training, education, and in-services.
• Participate in the Quality Assurance Program and related quality improvement activities.

Requirements

• Personal Support Worker certificate or equivalent.
• Minimum 1 year of experience in a related field.
• Demonstrated comprehensive knowledge of health care practices and techniques.
• Valid driver's license in good standing with a reliable vehicle for travel within HRH’s community.
• Current BCLS & CPR Certificates.
• Effective written and oral communication skills.
• Effective interpersonal skills to work as part of a multidisciplinary team.
• Knowledge, skill, judgment, and ability to work with patients in an understanding, sensitive, and compassionate manner.
• Self-directed individual with well-developed organizational skills.
• Ability to take initiative and work with minimal supervision.
• A positive attendance and work record is essential.
• Understanding of equity, diversity, and inclusion principles as they relate to health practices is an asset.
