About the Role

VHA Home HealthCare (VHA) is seeking a Registered Practical Nurse (RPN) to provide and coordinate home health nursing services to diverse individuals, families, and groups. This role is crucial in delivering high-quality, client-centered care within various community settings, emphasizing independence and well-being for clients.

Responsibilities

  • Assess, monitor, and plan care for clients.
  • Coordinate home health nursing services.
  • Evaluate care plan interventions through ongoing reassessment.
  • Ensure quality of care by identifying and acting on factors affecting practice autonomy.
  • Demonstrate professionalism, leadership, judgment, and accountability.
  • Support safe client care through continuous quality improvement.
  • Inform nursing supervisor of client activities, concerns, or changes.
  • Submit appropriate documentation to the case coordinator.
  • Maintain documentation standards according to the College of Nurses of Ontario.

Requirements

  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • RPN diploma/degree from a recognized university or college.
  • Valid First Aid or CPR Certification.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability.
  • Commitment to lifelong learning and professional growth.
  • Ability to apply critical thinking skills to provide client care.
  • A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is required.
