About the Role

The Addictions Counselor is responsible for maintaining a client caseload, identifying goals and objectives to improve emotional, social, and daily functioning. This role involves completing substance use disorder (SUD) screening and intake evaluations, crisis intervention assessments, and maintaining accurate electronic health records.

Responsibilities

  • Assist clients in creating and maintaining person-centered treatment plans every 90 days.
  • Attend weekly staff meetings, treatment teams, and supervision.
  • Complete mandatory documentation on EHR within established timelines.
  • Empower clients and encourage choice, demonstrating sensitivity to racial, cultural, ethnic heritage, and sexual orientation.
  • Provide linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs.
  • Refer clients and families to appropriate crisis management and/or behavioral health services.
  • Develop a comprehensive discharge plan in collaboration with clients, families, multidisciplinary staff, and referral agencies.
  • Provide individual, group, and/or family counseling for mental health and/or substance use.
  • Collaborate with other local agencies such as DSS, Probation, and Drug Court.
  • Complete urine drug screens on clients.
  • Comply with OMH, OASAS, New York State, and Federal regulations, laws, and ethical guidelines.
