About the Role
Supervise the delivery of services provided by Client Care Coordinators to enhance services for homeless families. This includes overseeing assessments, bio-psychosocial development, crisis intervention, counseling, client advocacy, and referrals to community resources. The role ensures achievable goals are implemented for permanent housing and improved permanency outcomes for clients.
Responsibilities
- Manage and supervise a team of Client Care Coordinators, including recruitment, training, supervision, evaluation, and scheduling.
- Guide client care coordinators in developing, implementing, and achieving program goals.
- Handle administrative, operational, and programmatic issues affecting client care coordinator staff.
- Assign cases to client care coordinators for newly admitted clients.
- Ensure timely completion of comprehensive bio-psychosocial assessments.
- Work effectively as part of a multi-disciplinary team to enhance client engagement and service delivery.
- Enhance the delivery and coordination of mental health and related services to clients.
- Ensure Client Care Coordinator reports clearly capture assessment findings and recommendations.
- Confer and consult with professional and technical personnel to implement a multidisciplinary approach to client care.
- Ensure Client Care Coordinators provide short-term counseling, outreach, crisis intervention, risk assessment, safety planning, and psycho-education.
- Collaborate with ACS and/or prevention services agencies and participate in conferences to advocate for clients.
- Ensure Client Care Coordinators provide appropriate referrals and linkages to mental health providers, government agencies, and community-based services.
- Serve as agency/program liaison between DHS, government agencies, and community-based organizations.
- Develop and implement group work for Client Care Coordinators to address family issues and enhance life skills.
- Ensure Client Care Coordinators complete all documentation in a timely manner.
- Ensure Client Care Coordinators assess clients for supportive housing, complete applications, and accompany residents to interviews.
- Ensure Client Care Coordinators complete and discuss exit summaries and provide referrals and supportive services.
- Participate in family meetings with case work and housing teams.
- Network and establish linkages with government and community-based organizations to create resources.
- Collaborate with the Director of Social Services and Housing Coordinator to ensure services lead to permanent housing placement.
- Complete special projects and assignments as needed.
- Work one late shift per week (11 am - 7 pm).
- Provide on-call crisis intervention support to Client Care Coordinators.
- Perform other duties as assigned.
- Manage, review, and monitor Client Care Coordinators’ maintenance of client records.
- Provide training to new and existing Client Care Coordinator staff.
- Provide individual and group supervision to Care Coordinators.
- Ensure high-risk cases are monitored closely and serviced effectively.
- Provide support and guidance on high-risk and challenging cases.
- Collect and analyze data on client services to evaluate team success.
- Provide Client Care Coordination services in the absence or vacancy of staff.
Requirements
- Must be a Licensed Clinical Social Worker (LCSW) in New York State upon hire or within three months of hire.
- Minimum of 3-5 years of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services.
- At least two years of supervisory experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services.
- Knowledge in three or more of the following areas: supervision of social workers, client assessment/service plan development/case management, child and adolescent development, parent-child relationships/family dynamics/diagnostic classifications, mental health and emotional/behavior health, Child Welfare policies and procedures, Government Entitlements (TANF, Social Security, Medicare), working with diverse cultures and ethnicities, strengths-based/solution-focused/family-centered practice, facilitating groups for adults.
- May be required to work long hours and a varied schedule.
- Traveling throughout various locations in the New York City area when necessary.
- Lifting and moving objects weighing up to 30 pounds several times a week.
- Must work one late a week from 11 am -7 pm to accommodate both staff and families.
- Provide on-call crisis intervention support to Client Care Coordinators.
Skills

- Team lead and provide effective supervision, guidance, and oversight of subordinates.

- Participate in and contribute to staff meetings and the development of policies and procedures.
- Participate in trainings and staff-development activities.
- Provide professional development to Client Care Coordinators.
- Work in collaboration with site leadership to provide training and consultation.
- Ensure that staff attend CORE trainings within (6) months of employment.
- Assess and respond to client needs.
- Collaborative working relationships with volunteers.
- Ensure team delivery of consistent high-quality services.
