About the Role

Provide skilled nursing and hospice care to maximize the comfort and health of patients, with a focus on patient involvement in medical decisions.

Responsibilities

  • Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
  • Assess, plan, implement, and evaluate nursing services consistent with the plan of care under the guidance of the After-Hours Nurse.
  • Communicate with attending physicians and other community resources to report changes and coordinate optimal care and resource utilization for the patient/family.
  • Maintain regular communication with Team Leaders regarding patient/family status and any unusual or potentially problematic issues.

Requirements

  • Must be bilingual in Cantonese.
  • Current license as a registered professional nurse in the State of California.
  • One year of experience as a professional nurse in the past three years OR a Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Experience in acute care, home health, and/or hospice setting preferred.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.
  • Must comply with all CDPH vaccine requirements for healthcare personnel.
