About the Role
This role assists older people in navigating the aged care assessment process by conducting thorough triage evaluations to identify the appropriate assessment pathway and prioritize their needs.
Responsibilities
- Establish rapport to create a safe and comfortable environment for the client to express their concerns and to confirm screening information utilising the Integrated Assessment Tool (IAT) Tool
- Determine eligibility and assessment priority and pathway
- Conduct risk assessments based on client vulnerability
- Gather information relevant to client circumstances and confirm the information within the referral
- Maintain currency of knowledge relating to the Aged Care Act and relevant Aged Care frameworks and standards
Requirements
- At least one year’s experience as a Triage or Clinical Delegate or Clinical Aged Care Assessor
- Bilingual – preferred languages are Mandarin, Cantonese, Vietnamese and Arabic
- Excellent Communication and Customer Service skills to both internal and external stakeholders
- Excellent planning, organising and problem-solving skills
- Effective negotiation, advocacy and conflict resolution skills
- Tertiary qualifications – in a health-related discipline directly related to health, aged care or related specialist area. E.g. occupational therapists, physiotherapists and registered nurses
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
- Willingness to apply for a Police Check

Salary/Compensations
- $56.73 per hour + superannuation
