About the Role

This role requires a clinical qualification/background and involves conducting assessments to identify and meet the complex needs of clients, referring them to appropriate aged care services. The Aged Care Assessment Services (ACAS) team operates independently and plays a pivotal role in ensuring Older Australians are assessed for the care and services they truly need.

Responsibilities

• Conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care ACT
• Collaborating with clients, families and multidisciplinary teams as required
• Working collaboratively with colleagues to promote a supportive and positive team culture
• Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services
• Reviewing support plans to meet client’s changing support needs
• Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes
• Maintaining currency of knowledge relating to aged care services both funded and non-funded
• Assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes

Requirements

• Tertiary Qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker)
• Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
• An Australian Driver’s license and access to a registered, insured vehicle
• Willingness to apply for a Police Check

Skills

• Bilingual - Cantonese or Mandarin ideally (preferred but not essential)
• Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons including to people living with dementia (desirable)

Salary/Compensations

• $54.47 per hour + superannuation
