About the Role
Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Learning Disabilities team, based in Havering. This role focuses on managing a diverse caseload, working closely with individuals and stakeholders to deliver positive outcomes, and promoting independence.
Responsibilities
- Manage a diverse caseload of adults with learning disabilities, including complex cases depending on experience.
- Work closely with adults, carers, healthcare professionals, and community partners to deliver positive and lasting outcomes.
- Undertake person-centred assessments, reviews, and support planning.
- Complete safeguarding enquiries and risk assessments to protect vulnerable adults.
- Promote independence and ensure individuals receive the right support at the right time.
- Support safe hospital discharges and work to prevent avoidable admissions.
- Develop and review personalised care and support plans.
- Work collaboratively within a multi-disciplinary team alongside health and social care professionals.
- Maintain accurate and timely case records and reports.
- Contribute to service development and continuous improvement initiatives.
- Provide mentoring or supervision support to colleagues where appropriate.
Requirements
- A recognised Social Work qualification
- Current Social Work England (SWE) registration
- Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
- Good knowledge of the Mental Capacity Act and relevant legislation
- Strong multi-agency partnership working skills
- Excellent communication, organisational, and problem-solving abilities
- A compassionate, proactive, and person-centred approach to practice
- A commitment to delivering high-quality outcomes for adults and their families
