About the Role

This role plays a key part in advancing health equity by working closely with Black communities to improve access to preventive care, health education, screening, and system navigation. Responsibilities

Engage in culturally responsive engagement and outreach to individual community members and provide accurate and relevant information/education about preventive health care, cancer screening, vaccination, chronic disease prevention and management, mental health promotions, access to primary care, and other local population health challenges.
Conduct culturally and linguistically responsive educational presentations individually and within groups in the communities.
Engage community members in preventive care conversations and health action planning.
Connect community members to services and programs that support the determinants of health, facilitating referrals for primary care and cancer screening.
Conduct grassroot community outreach activities, including in-person and virtual/social media.
Participate in relevant committees and service networks internally and externally.
Collect, track, and help analyze statistics for trending, reporting, and program evaluations.
Actively participate in the development, implementation, and evaluation of health promotion and wellness clinics.
Perform other duties as assigned in support of operations.

Requirements

Post-secondary education in a relevant field (e.g., health promotion, human services) is an asset.
Appreciation for a combination of lived/living and work experience relevant to the role.
Experience with community-based outreach in an employment or volunteer capacity.
Valid Ontario Driver's license and access to a vehicle.
Demonstrated current expertise/experience in EDI and anti-racism practices, integrated primary care and management, chronic disease prevention and management, health promotion and wellness.
Fluency in a second language that reflects the communities served.
Strong understanding of social services, community work, community resources, and equity-based population health approaches.
Ability to work independently and collaborate effectively in an interdisciplinary team environment.
A valid vulnerable sector police record check completed within one year.

We serve over 15,000 clients a year, including seniors, adults, and youth with various challenges.
The Carefirst Community Health Team supports population health by bringing preventive care, health education, screening, and system navigation directly into the community, with a focus on equity and early intervention.
The team helps identify unmet needs, reduce barriers to care, and connect individuals to health and social supports.
Programs include Preventive Care Clinic, Cancer Prevention Program, Mobile Health Clinics, and Black Health Initiative, aiming to improve equitable access to care and promote wellness.
