About the Role
Graham provides life-changing tools and resources for children, young adults and families facing obstacles caused by poverty, racial injustice, and lack of access to educational opportunities, living wage employment, quality healthcare, and affordable housing. We collaborate with communities and partners to create and implement innovative strategies for healthy, joyful, and successful lives. Our Community Mental Health & Wellness Clinic is an Article 31 OMH program offering evidence-based treatment for children, youth, young adults, and their families (ages 5-25).
Responsibilities
- Provide individual, group, family, and collateral treatment and crisis services/interventions that are focused and goal-oriented.
- Offer information and counseling on the best course of action during sessions.
- Apply appropriate evidence-based psychotherapeutic interventions to assess, monitor, and ameliorate symptoms, and minimize negative effects of trauma symptoms, behaviors, and environmental stressors.
- Provide, implement, and coordinate comprehensive initial and ongoing assessments and treatment plans for new intake clients and ongoing clients.
- Collaborate with clients, collaterals, and their multi-disciplinary clinical treatment team in developing, reviewing, and modifying treatment plans.
- Monitor and evaluate clients’ progress and modify treatment plans as needed.
- Consult with other behavioral health, primary care, and specialty care providers as appropriate.
- Collaborate with outside organizations such as schools, hospitals, higher level of care programs, and other service programs on behalf of client’s care needs.
- Provide engagement and rapport to foster an individual’s commitment to engage in a therapeutic relationship and attend therapy regularly.
- Explore motivation, resistance to therapy, and attendance as needed.
- Maintain electronic records and documentation according to OMH, Medicaid regulations, and Graham policies.
- Maintain expected levels of productivity and provide quality care.
- Provide crisis intervention services to de-escalate situations of acute distress or requiring immediate attention.
- Manage and report incidents, and complete necessary documentation.
- Coordinate the discharge planning process.
- Identify resources and support needed for transition to another program and make necessary referrals and linkages for treatment, rehabilitation, and support services.
- Be actively self-reflective and self-aware of how clinical and administrative practice affects clients, families, and communities.
- Be open and willing to receive constructive feedback.
- Approach work with an anti-racism lens and be open to continuous learning and professional growth.
- Participate in clinical meetings, staff meetings, trainings, supervision, and case conferences.
- Maintain professional behaviors and ethical standards.
- Uphold the privacy of patient, client, or protected member health information.
Requirements
- Master’s degree with professional license: LMSW, LCSW, LMHC, LMFT, LCAT.
- Limited Permit holders eligible.
- MSW permit holders must obtain a license within 1 year.
- MHC permit holders must obtain a license within 2 years.
- 1 year of post-masters experience of clinical experience with children/adolescents/families.
- Able to work evenings and/or Saturdays.
- High degree of computer literacy.
- Experience in an Article 31 Mental Health Clinic preferred.
- Demonstrated experience with diverse populations preferred.

- Clinician I Unlicensed MSW, MHC Non-Exempt, Hourly $54,918.00
- Clinician II LMSW/Limited Permit Non-Exempt, Hourly $58,248.68
- Clinician III LCSW, LMHC, LMFT, or LCAT Exempt, Salary $64,254.06

- Graham Windham’s Community Mental Health & Wellness Clinic is an Article 31 OMH program that offers a variety of evidence-based treatment for children, youth, young adults, and their families from the age of 5-25 years of age, based on our comprehensive psychiatric and biopsychosocial evaluations.
- We offer individual therapy, family therapy, group therapy, psychological and psychiatric evaluations, psychopharmacology management, and support services.
- Our Harlem clinic offers walk-in hours for immediate mental health assessments.
- Each treatment plan is created in collaboration with the client based on the needs of the individual client.
- We also offer bilingual services in Spanish to better support the needs of our communities.
