About the Role

The Registered Nurse (RN) will work closely with the interprofessional ACT team providing nursing care, health promotion, and assistance with daily living and support to individuals with serious and persistent mental illness in a community setting. The role involves psycho-education, mental status assessment, physical health monitoring, developing therapeutic alliances, and linking clients to appropriate resources.

Responsibilities

  • Provide nursing care, health promotion, and assistance with activities of daily living.
  • Provide psycho-education around medications and symptoms.
  • Assess mental status and monitor physical health needs.
  • Develop therapeutic alliances with clients and their support systems.
  • Work from a recovery-based perspective.
  • Assess client needs and link to appropriate resources.
  • Monitor treatment regimes.
  • Support the agency’s primary health care and chronic disease management initiative.

Requirements

  • Undergraduate degree in nursing.
  • Registration and good standing with College of Nurses of Ontario as a Registered Nurse.
  • Minimum of two (2) years’ experience working with people with mental illness.
  • Sound mental health assessment and teaching skills.
  • Medication monitoring skills.
  • Case management skills.
  • Crisis intervention skills.
  • Knowledge and understanding of the ACT Model and recovery-oriented practice.
  • Valid Ontario Driver’s License.
  • Access to a vehicle and insurance coverage.
  • Ability to adhere to COVID-19 policy and provide proof of vaccination.

Work Type

  • Full Time Permanent
  • Varying schedule including weekdays, weekends, holidays, and occasional evenings

Experience Level

  • Minimum of two (2) years’ experience working with people with mental illness.

Education Level

  • Undergraduate degree in nursing.
  • Registered Nurse (RN) designation.

Salary/Compensations

  • $68,975 - $77,911
