About the Role
Supervise the delivery of services provided by Client Care Coordinators to enhance services for homeless families. This includes overseeing assessments, bio-psychosocial development, crisis intervention, counseling, client advocacy, and referrals to community resources. The role ensures achievable goals are implemented for permanent housing and improved permanency outcomes for clients in shelter.
Responsibilities
- Manage and supervise a team of Client Care Coordinators, including recruitment, training, supervision, evaluation, and scheduling.
- Guide client care coordinators in developing, implementing, and achieving program goals.
- Address administrative, operational, and programmatic issues affecting client care coordinator staff.
- Assign cases to client care coordinators for newly admitted clients.
- Ensure timely completion of comprehensive bio-psychosocial assessments for each client.
- Collaborate effectively within a multi-disciplinary team to enhance client engagement and service delivery.
- Improve the delivery and coordination of mental health and related services to clients.
- Ensure Client Care Coordinator reports clearly document assessment findings and recommendations.
- Consult with professional and technical personnel to implement a multidisciplinary approach to client care.
- Ensure Client Care Coordinators provide short-term counseling, outreach, crisis intervention, risk assessment, safety planning, and psycho-education.
- Collaborate with ACS and/or prevention services agencies and participate in conferences to advocate for clients.
- Ensure Client Care Coordinators provide appropriate referrals and linkages to mental health providers, government agencies, and community-based services.
- Serve as agency/program liaison between DHS, government agencies, community-based organizations, and groups.
- Develop and implement group work for Client Care Coordinators to address family issues and enhance life skills.
- Ensure Client Care Coordinators complete all documentation in a timely manner.
- Ensure Client Care Coordinators assess clients/families for supportive housing, complete applications, and accompany residents to interviews.
- Ensure Client Care Coordinators complete and discuss exit summaries with clients and provide referrals and supportive services for their new community.
- Participate in family meetings with case work and housing teams to discuss planning efforts and progress.
- Network and establish linkages with government and community-based organizations to create resources.
- Collaborate with the Director of Social Services and Housing Coordinator to ensure services lead to permanent housing placement.
- Complete special projects and assignments as needed.
- Work one late shift per week (11 am - 7 pm).
- Provide on-call crisis intervention support to Client Care Coordinators.
- Perform other duties as assigned.
- Manage, review, and monitor Client Care Coordinators’ maintenance of client records, ensuring quality assurance standards are met.
- Provide training to new and existing Client Care Coordinator staff.
- Provide individual and group supervision to Care Coordinators, offering coaching, guidance, and support.
- Ensure high-risk cases are monitored closely and serviced effectively.
- Provide support and guidance on high-risk and challenging cases.
- Collect and analyze data on client services to evaluate team success.
- Provide Client Care Coordination services in the absence or vacancy of staff.
- Facilitate effective communication with other staff in an interdisciplinary context.
- Participate in staff meetings and contribute to policy and procedure development.
- Participate in trainings and staff-development activities to acquire and maintain necessary skills.
- Provide professional development to Client Care Coordinators to enhance assessment skills.
- Collaborate with site leadership to provide training and consultation to the case management team.
- Ensure staff attend CORE trainings within six months of employment.
Requirements
- Must be a Licensed Clinical Social Worker (LCSW) in New York State upon hire or within three months of hire.
- Minimum of 3-5 years of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services.
- At least two years of supervisory experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services.
- Knowledge in three or more of the following areas: supervision of social workers, client assessment/service plan development/case management, child and adolescent development, parent-child relationships/family dynamics/diagnostic classifications, mental health and emotional/behavioral health, Child Welfare policies and procedures, Government Entitlements (TANF, Social Security, Medicare), working with diverse cultures and ethnicities, strengths-based/solution-focused/family-centered practice, facilitating groups for adults.
- May be required to work long hours and a varied schedule.
- May be required to travel throughout various locations in the New York City area.
- May be required to lift and move objects weighing up to 30 pounds several times a week.
