About the Role

Quadrant Health Group is seeking a passionate and dedicated Behavioral Health Case Manager to join their growing team. This role is crucial in empowering individuals with mental health and/or substance use disorders to achieve their personal and therapeutic goals.

Responsibilities

  • Assist and support residents through the entire treatment experience from admission to discharge.
  • Monitor the course of treatment proactively to minimize barriers to treatment and recovery.
  • Contribute insightful, practical, and meaningful information to the treatment planning process through groups and individual assessments.
  • Orient residents to program rules, expectations, intent, and the daily treatment schedule.
  • Assist clients with creating individualized treatment plans outlining goals and objectives.
  • Conduct comprehensive assessments to identify client needs and develop individualized care plans.
  • Coordinate and monitor the delivery of behavioral health services, including therapy, medication management, and support groups.
  • Connect clients with community resources, such as housing, food assistance, employment services, and transportation.
  • Develop the Comprehensive Treatment Plan with client input and participation in treatment planning meetings.
  • Advocate for clients' rights and needs with healthcare providers, social service agencies, and other organizations.
  • Provide crisis intervention and support as needed.
  • Maintain accurate and timely documentation of client interactions and progress.
  • Collaborate with interdisciplinary team members, including therapists, psychiatrists, and other healthcare professionals.
  • Contact residents’ family members to set appointments or educate them on the residents’ current status and/or progress.
  • Monitor client progress and adjust care plans as needed.
  • Participate in team meetings and training sessions.
  • Maintain compliance with all applicable regulations and ethical standards.

Requirements

  • Bachelor's degree in social work, psychology, counseling, or a related field (Master's preferred).
  • Minimum of 3 years of experience in behavioral health case management.
  • Experience working with diverse populations.
  • Proficiency in electronic health records (EHR) systems.
  • Current CPR and First Aid Certification.
  • Successful completion of Pre-Employment Requirements including a criminal background clearance, drug testing, and health screening.

Skills

  • Knowledge of mental health and substance use disorders and treatment modalities.
  • Strong understanding of community resources and social service systems.
  • Demonstrates an understanding of computer technology, appropriate for the role.
  • Written and verbal communication techniques commensurate with the position’s skill level.
  • Demonstrates working knowledge of State and Accreditation requirements: DHCS – Substance Use Disorders, JCAHO and regulatory agencies.
  • Attuned to clinical protocol of mind, body spirit.
  • Knowledge of the philosophy and approach of the 12-step model of recovery (AA, CA, NA, Alanon).
  • Fully understands and maintains policies regarding professional ethics, including appropriate boundaries and patient confidentiality.
  • Basic knowledge of referrals, both in and out of the organization.
  • Ability to communicate clearly, with clients, staff, peers, supervisors and resources.
  • Strong clinical and assessment skills.
  • Strong organizational and time-management skills.
