About the Role
DaVita is seeking a Licensed Practical Nurse for an outpatient dialysis center to provide care to patients with end-stage renal failure or chronic kidney disease.
Responsibilities
- Deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.
- Work with your head, heart and hands each day in a fast paced environment.
Requirements
- Current LPN-LVN license in the state of practice required
- Current CPR certification required
- IV certification required if required by state regulations
- High school diploma or equivalent required
- Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system
- BONENT or CHT certification preferred or as required by state regulation
- Minimum of one year's experience in a hospital or clinical setting preferred
- Basic computer skills and proficiency in MS Word and Outlook required
- Functional proficiency in computer systems of DaVita within 90 days of hire required

Salary/Compensations
- $30.00 - $44.00 per hour
- Licensed Practical Nurse ICHD Experienced: $33.75 - $44.00 per hour
- New York Exempt: New York City and Long Island: $66,300.00/year, Nassau, Suffolk, and Westchester counties: $66,300.00/year, Remainder of New York state: $62,353.20/year
- New York Non-exempt: New York City and Long Island: $17.00/hour, Nassau, Suffolk, and Westchester counties: $17.00/hour, Remainder of New York state: $16.00/hour
