About the Role
This is a part-time position in the Admissions Department, requiring weekend availability and holiday rotation. The Licensed Practical Nurse (LPN) Coordinator manages the daily clinical functions of the team, supports the patient care plan implementation, and acts as a liaison between staff, patients, and external agencies. Responsibilities may include direct patient care and record maintenance.
Responsibilities
- Assists in coordination and implementation of the patient plan of care.
- Handles phone calls from the patient, primary care giver, or designee regarding any questions or concerns.
- Collaborates with the interdisciplinary teams, RNs, or Team Manager.
- Coordinates and schedules patient visits.
- Provide timely documentation/data entry per Hospice time frames and deadlines.
- Communicates current health status and needs of patients to appropriate team professionals.
- Assists the team and Team Manager with other administrative functions, as necessary.
Requirements
- 1 year of nursing practice as a Licensed Practical Nurse preferred.

- Current LPN licensure in the State of New York
