About the Role
This is a full-time agency role supporting adults with learning disabilities to live independently, remain safe, and access the services they need to achieve positive outcomes.
Responsibilities
- Manage a caseload of adults with learning disabilities, including complex cases.
- Undertake person-centred assessments, reviews, and support planning.
- Complete safeguarding enquiries and risk assessments.
- Support safe hospital discharges and prevent avoidable admissions.
- Develop and review personalised care and support plans.
- Work collaboratively within a multi-disciplinary team alongside health and social care professionals.
- Maintain accurate and timely case records and reports.
- Contribute to service development and continuous improvement initiatives.
- Provide mentoring or supervision support to colleagues where appropriate.
Requirements
- A recognised Social Work qualification.
- Current Social Work England (SWE) registration.
- Experience undertaking assessments, reviews, and safeguarding interventions within adult social care.
- Good knowledge of the Mental Capacity Act and relevant legislation.
- Strong multi-agency partnership working skills.
- Excellent communication, organisational, and problem-solving abilities.
- A compassionate, proactive, and person-centred approach to practice.
- A commitment to delivering high-quality outcomes for adults and their families.
