About the Role
The Field Care Supervisor role supports the delivery of high-quality care services through field functions, including assessing and reviewing individual services, and supervising, instructing, and supporting care workers.
Responsibilities
- Support the delivery of the highest quality care support services.
- Carry out assessment and review of individual services.
- Provide practical supervision, instruction, and support of care workers.
- Work with Care Coordinators to support the planning of effective care worker rotas.
- Carry out regular reviews of individual services, including needs and risk assessments.
- Participate in multi-disciplinary reviews of service users’ needs.
- Shadow new care workers in the field to ensure competence.
- Carry out on-site observation and evaluation of care worker practice.
- Spend the majority of working time in the community carrying out primary functions.
- Undertake office-based administrative work.
Requirements
- Experience in carrying out initial risk and needs assessments in respect of individual service users.
- Ability to work in partnership with service users, their family, carers, advocates, healthcare professionals and others to ensure seamless delivery of holistic packages of individual care support.
