About the Role
An Adult Social Worker is needed to join the Adult’s Review Team in West London. This role is crucial in promoting the well-being and independence of individuals, making a significant impact on their lives.
Responsibilities
- Undertake assessments and reviews under the Care Act 2014
- Hold a caseload
- Support the duty function
- Evaluate the needs of individuals, ensuring they receive appropriate care and support
- Conduct thorough assessments
- Develop care plans
- Regularly review the progress and effectiveness of care plans
- Collaborate with other professionals and agencies to provide comprehensive support
Requirements
- A recognised Social Work qualification
- Current Social Work England (SWE) registration
- Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
- Good knowledge of the Mental Capacity Act and relevant legislation
- Strong multi-agency partnership working skills
- Excellent communication, organisational, and problem-solving abilities
- A compassionate, proactive, and person-centred approach to practice
- A commitment to delivering high-quality outcomes for adults and their families
