About the Role

Provides physical therapy and related services for patients in an Assisted Living, Independent Living, and Memory Care setting, ensuring quality patient care and maintaining positive interactions with communities and clients.

Responsibilities

  • Screen residents as appropriate.
  • Evaluate and treat patients referred by state-approved sources.
  • Comply with state and federal guidelines for accurate and timely billing and documentation.
  • Establish and maintain effective, patient-centered interactions with patients and the interdisciplinary team.
  • Refer patients for treatment to Certified Physical Therapy Assistants (PTAs).
  • Provide consultation and clinical supervision to PTAs.
  • Maintain availability to answer questions from PTAs.
  • Directly supervise PTA and Rehab Tech in skilled tasks, promoting efficient resource utilization and clinical skill development.
  • Integrate physical therapy's role into community and interdisciplinary treatment teams.
  • Contribute to community patient care, utilization review, case management, and family conference activities.
  • Establish and maintain good working relationships with other healthcare professionals and colleagues.
  • Make appropriate referrals to other members of the rehabilitation team.
  • Develop and implement new programs and techniques to enhance rehabilitation services.
  • Adhere to environmental safety, risk management, infection control, and emergency preparedness policies.
  • Adhere to patient rights and privacy requirements.
  • Enhance clinical expertise and professional skills through continuing education.
  • Maintain awareness of current issues in physical therapy and healthcare.
  • Understand various payment models, billing and treatment guidelines, and clinically appropriate means of delivery.
  • Provide quality care with reasonable goals and outcomes.
  • Be compliant with infection control procedures and environmental safety protocols.
  • Perform patient lifting using appropriate biomechanical techniques, capable of lifting fifty pounds of dead weight alone.
  • Maintain confidentiality of all information.
  • Conduct self in an ethical, legal, and responsible manner.
  • Adhere to the Team Member Handbook and Code of Business Conduct.
  • Travel between communities during scheduled hours.
  • Work varying hours and shifts, including weekends and holidays.
  • Attend all mandatory meetings, trainings, and assignments.
  • Perform other duties and responsibilities as assigned.

Requirements

  • Graduate of an APTA accredited school of physical therapy.
  • Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.).
  • Basic Internet navigation skills.
  • Supervise the work of one or more Physical Therapist Assistants (PTAs), Rehab Techs, and/or Physical Therapy students.
  • Provide consultation and clinical supervision; create and ensure treatment plans are executed.
  • Assist the RDO and DOR with clinical development of PTAs and rehab techs.
  • Must be capable of lifting fifty (50) pounds of dead weight alone.
  • Must be able to travel between communities during scheduled hours.
  • Must be able to work varying hours and shifts, including weekends and holidays.
