About the Role

This role is based in an Assisted Living, Independent Living, and Memory Care setting. The Physical Therapist provides physical therapy and related services for patients while maintaining positive interactions with communities and clients, adhering to Reliant Rehabilitation guidelines. Responsibilities include screening, evaluation, treatment planning, discharge planning, supervision of assistants and technicians, and case management.

Responsibilities

  • Optimize patient functionality and skills through physical therapy services.
  • Screen residents as appropriate.
  • Evaluate and treat patients referred by state-approved sources.
  • Comply with state and federal guidelines for accurate and timely billing and documentation.
  • Establish and maintain effective, patient-centered interactions with patients and the interdisciplinary team.
  • Refer patients for treatment to Certified Physical Therapy Assistants (PTAs).
  • Provide consultation and clinical supervision to PTAs.
  • Maintain availability to answer questions from PTAs.
  • Directly supervise PTA and Rehab Tech in skilled tasks, promoting efficient resource utilization and clinical skill development.
  • Build a strong physical therapy program by integrating therapy's role into the community and interdisciplinary team.
  • Contribute to community patient care, utilization review, case management, and family conferences.
  • Establish and maintain good working relationships with other healthcare professionals and colleagues.
  • Make appropriate referrals to other members of the rehabilitation team.
  • Develop and implement new programs/techniques to enhance rehabilitation services.
  • Adhere to Reliant Rehabilitation and client-specific policies for environmental safety, risk management, infection control, and emergency preparedness.
  • Adhere to patient rights and privacy requirements.
  • Enhance clinical expertise, professional, and management skills through continuing education and self-study.
  • Maintain awareness of current issues in physical therapy and healthcare.
  • Understand various payment models, billing and treatment guidelines, and clinically appropriate delivery methods.
  • Provide quality care with reasonable goals and outcomes.
  • Be compliant with infection control procedures and environmental safety protocols.
  • Perform patient lifting using appropriate biomechanical techniques, capable of lifting fifty (50) pounds of dead weight alone.
  • Maintain confidentiality of Reliant Rehabilitation, community, and patient information.
  • Conduct self in an ethical, legal, and responsible manner.
  • Adhere to policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct.
  • Travel between communities during scheduled hours.
  • Work varying hours and shifts, including weekends and holidays.
  • Attend all mandatory meetings, trainings, and assignments.
  • Perform other duties and responsibilities as assigned.

Requirements

  • Graduate of an APTA accredited school of physical therapy.
  • Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.).
  • Basic Internet navigation skills.
  • Supervise the work of one or more Physical Therapist Assistants (PTAs), Rehab Techs, and/or Physical Therapy students.
  • Provide consultation and clinical supervision to PTAs and Rehab Techs.
  • Create and ensure treatment plans are executed.
  • Assist with clinical development of PTAs and Rehab Techs.
  • Effective clinical problem-solving skills.
  • Be a strong team player.
  • Strong verbal and written communication skills.
  • Ability to build strong relationships with a diverse population.
  • Ability to read, write, speak, and comprehend in English: instructions, correspondence, charts, memos, and reports.
  • Ability to work in a fast-paced, productive work environment.
