About the Role

Responsibilities
- Provide home visits for patient assessment, treatment, and support, including pronouncement of patient death, crisis management, and admission.
- Plan comprehensive services for patients and families, developing and revising nursing care plans with physician and IDG input.
- Utilize excellent customer care skills with internal and external customers, including proper phone etiquette.
- Prepare clinical and progress notes demonstrating progress toward goals.
- Inform physician and other personnel of patient needs and intervention outcomes, evaluating patient/family response to care.
- Ensure timely submission of all documentation.
- Perform other duties as required.
- Ensure compliance with local, state, and federal laws, Medicare regulations, and personnel policies.
Requirements
- Graduate of an accredited School of Nursing (Diploma, Associate, or Baccalaureate).
- Current State Registered Nurse License.
- Current Driver's License.
- Reliable transportation.
- Ongoing valid and current auto liability insurance.
- Two years of RN experience preferred in a healthcare/clinical setting.
