About the Role
Springboard's mission is to build stronger communities by helping youth and adults develop the skills they need to reach their full potential. This role supports a living program for transitional aged youth (TAY) with developmental disabilities, assisting them in transitioning from the child welfare system and developing life skills for independent living.
Responsibilities
- Collaborate with individuals with developmental disabilities to develop goals, goal plans, and strategies to support those goals.
- Conduct needs analysis in collaboration with the individual supported to identify unique needs.
- Support the delivery of services and supports according to goal plans that foster an effective transition to independent living.
- Support Senior DSW's in locating e-programs, services, housing, and employment opportunities.
- Work effectively with TAY staff to develop strategies and implement therapeutic programming.
- Ensure appropriate personal and physical care, crisis response, counselling, and emotional support are provided in a trauma-informed manner.
- Support the development and implementation of life skills facilitation programs.
- Ensure program plans promote independent living, are based on individual needs/goals, and reflect a high level of learning.
- Facilitate life skills programming, one-on-one or in groups, based on individual needs, goals, and support plans.
- Participate in assessing the long-term service needs of individuals supported and the program.
- Provide supports for individuals to enable active participation in community activities and/or recreational outings.
- Plan community outings, appointments, and activities in accordance with individual needs, safety protocols, resources, and programming.
- Reduce safety incidents through strategies that reinforce positive behaviour and identify behaviour patterns.
- Apply Individual Behaviour Support Plans and Safe Management in case of crisis or challenging behaviour.
- Ensure written materials and/or correspondence are professional, clearly written, and reflect agency philosophy and individual privacy.
- Monitor finances for individuals supported, where required.
Requirements
- Post-secondary diploma/degree in Social Services and/or Developmental Services or a non-related degree/diploma with two years' experience in developmental services.
- Valid CPR/First Aid and Crisis Prevention Institute (CPI) certifications.
- Strong knowledge in computer applications (MS Word, Excel, PowerPoint, Outlook).
- Minimum two (2) years of recent work-related experience working directly with individuals with a developmental disability or dual diagnosis.
- Experience with case management.
- Experience supporting youth involved in the child welfare system.
- Experience working from a trauma-informed and harm reduction approach.
- Direct experience engaging with diverse populations and ability to apply a culturally responsive lens.
- Understanding of the Social Inclusion Act and the Act for the Supports of Persons with Developmental Disabilities (2008).
- Working knowledge of the Developmental Services Sector.
- Employment requires satisfactory VSS, references, and proof of education.
