About the Role

The Developmental Services' TAY home is a supported living program providing transitional aged youth (TAY) with developmental disabilities support for their transition from the child welfare system. This role assists individuals in developing life skills pertinent to their life stage, offering a unique and innovative approach to skills training in an inclusive and supportive home environment.

Responsibilities

• Locate programs, services, housing, and employment opportunities for individuals supported that align with goal plans.
• Coordinate and provide direct support in the implementation of goals, monitoring achievements and outcomes.
• Work effectively with other TAY staff to develop strategies and implement therapeutic programming.
• Receive direction from Senior Developmental Service Workers to support individuals in the home achieve goals.
• Ensure appropriate personal and physical care, crisis response, counselling, and emotional support are provided in a trauma-informed manner.
• Maintain documentation and monitor finances for supported individuals.
• Support the development of programs both within and outside of the home.
• Implement life skills facilitation to enable individuals to live and integrate into the community independently.
• Ensure program plans promote independent living based on individual needs/goals and are developed in collaboration with the Supervisor.
• Facilitate life skills programming, one-on-one or in groups, based on individual needs, goals, and support plans.
• Participate in assessing the long-term service needs of individuals supported and the program.
• Oversee and implement menu planning and meal preparation in collaboration with individuals living in the home.
• Provide support for individuals to enable them to actively participate in community activities and/or recreational outings safely.
• Plan community outings, appointments, and activities in accordance with individual needs, safety protocols, available resources, and programming to promote social inclusion.
• Demonstrate professionalism and respect at all times, fostering positive relationships with individuals, their families/caregivers, and the community.
• Reduce safety incidents through strategies that reinforce positive behaviour and identify behaviour triggers.
• Apply Individual Behaviour Support Plans and Safe Management in case of crisis or challenging behaviour.
• Ensure the safety and security of the house, individuals, and staff as per legislation and policies.
• Identify and report any potential hazards swiftly and appropriately.
• Ensure written materials and correspondence are professional, clearly written, and reflect the agency’s philosophy and individual privacy.
• Support staff scheduling as needed.
• Regularly, accurately, and timely update files, databases, or reports.
• Use applicable computer systems proficiently (Outlook, Team, MS Word, PowerPoint, Excel) for presentations, program materials, correspondence, reports, and file updates.

Requirements

• Post-Secondary school diploma in Social Services or Developmental Services or Secondary school diploma with at least one year of experience in Developmental Services.
• Valid First Aid and Safe Management certifications.
• Minimum one (1) year of recent work-related experience working directly with individuals with a developmental disability or dual diagnosis.
• Minimum one (1) year of recent work-related experience with case management.
• Minimum one (1) year of recent work-related experience supporting youth involved in the child welfare system.
• Minimum one (1) year of recent work-related experience working from a trauma-informed and harm reduction approach.
• Minimum one (1) year of recent work-related experience engaging with diverse populations and applying a culturally responsive lens.
• Understanding of the Social Inclusion Act and the Act for the Supports of Persons with Developmental Disabilities (2008).
• Working knowledge of the Developmental Services Sector.
• Employment requires satisfactory VSS, references and proof of education.

Skills

• Proven individual and group facilitation skills leading to increased independence related to activities of daily living.
• Assessment skills for service delivery and operational/programming issues, including suitability and risk determination.
• Program planning skills to implement, evaluate, and modify programs.
• Effective crisis intervention skills to diffuse challenging situations.
• Ability to work well under pressure to ensure minimal interruption in service delivery.
• Written and verbal communication skills in English.
• Strong knowledge in computer applications, i.e., MS Word, Excel, PowerPoint, Outlook.
• Ability to communicate in French verbally and in written form (Nice to Have).
