About the Role

The Case Coordinator is responsible for developing and supporting individual support plans for clients with developmental disabilities receiving residential support from the Family Home Program. This role involves reviewing and assessing client needs, providing comprehensive assessments, and developing care plans in consultation with clients and caregivers. The Case Coordinator supports client self-management within VON policies and procedures.

Responsibilities

  • Coordinate and provide Family Home support for a caseload in accordance with program criteria, guidelines and directives.
  • Ensure referrals and support are provided in emergency and urgent situations.
  • Participate in the screening and matching of new Family Home Providers.
  • Interpret and complete current standardized assessment tools such as the interRAI CHA, Supports Intensity Scale, Application for Developmental Services, or other assessment tools as appropriate.
  • Complete client assessments using conversation-based methodology.
  • Develop a comprehensive therapeutic rapport.
  • Identify and prioritize environmental risks for client’s health and safety.
  • Develop a comprehensive care plan, interpreting the Clinical Assessment Protocol and care planning.
  • Facilitate access to client-identified services and navigate to appropriate resources.
  • Make referrals to community and health resources as required.
  • Consult with partners in care service plans to ensure shared understanding of client needs and appropriate interventions/services.
  • Monitor client progress and adjust care plans as required.
  • Review client care plan expenditures and support clients to manage within the client care budget, advocating for additional financial resources when needed.
  • Access and review assessments.
  • Collect and report relevant data to the Manager on program outcomes as required by the funder.
  • Adhere to prescribed privacy policies and standards for Community Support Service or Developmental Services agencies including VON.
  • Positively and professionally represent all potential providers, external stakeholders, community agencies, and service providers to clients while maintaining a depth of knowledge on community resources.
  • Mediate and problem-solve with multiple parties in a variety of contexts.
  • Promote the goals and values of VON and their role as an integrated community care provider.
  • Promote a safe and healthy workplace in accordance with the provincial Occupational Health and Safety Act and Regulations and compliant with the VON Safety Management System.
  • Abide by all VON policies and work practices.
  • Abide by all confidentiality and protection of personal information policies, regulations, and practices and ensure appropriate safeguards are in place.
  • Work in collaboration with other staff in a team approach to service delivery.

Requirements

  • University Degree or College Diploma in a Health or Social Services field.
  • Minimum of 3 years related experience in a home and community care organization or social services program.
  • Experience in case management.
  • Experience in building external and community relationships.
  • Demonstrated experience in completing validated common assessment tools and maintaining competency.
  • Broad working knowledge of community resources, including services and programs within the defined catchment area.
  • Knowledge and skills in comprehensive assessment and the ability to maintain proficiency.
  • Working with volunteers and seniors is an asset.
  • Strong attention to detail and accuracy.
  • Skilled in communication (verbal, written, listening), rapport building, and relationship management.
  • Computer proficiency in Windows OS and Microsoft Office programs.
  • Ability to use analytical skills and understand/interpret client assessment data.
  • Ability to effectively manage multiple tasks, prioritize, meet deadlines, and work well under pressure.
  • Knowledge of social and developmental service sectors may be required.
  • Demonstrated skills in leadership and team building.
  • Ability to educate clients and families on available program resources.
  • Strong time management and organizational skills.
  • Ability to work independently and as part of a team.

  • Ability to maintain confidentiality.
  • Must be able to work flexible hours to meet client and family needs, including evenings and weekends.
  • Ability to respond to emergency client situations 24/7 (on call).
  • Available for weekend assessment coverage.
  • Must have access to a personal vehicle for business use.
  • Possess a current driver’s license and proof of vehicle insurance.
  • A current and original copy of a satisfactory Criminal Records Check.
  • A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
  • The use of Personal Protective Equipment (PPE) may be required.
  • Requires a detail-oriented approach in a dynamic environment.
  • Requires physical activity including lifting, carrying, bending, reaching, kneeling, and other movements.
  • Individuals are required to walk, sit, stand, and climb stairs throughout the day.
  • Some tasks require fine hand movements.
