About the Role
All Heart Homecare Agency is seeking dedicated Licensed Practical Nurses (LPNs) to provide one-to-one private duty nursing care in patients’ homes. This role offers flexibility, professional growth, and the chance to make a meaningful difference in patients' lives, with stable, consistent case assignments and per diem options available.
Responsibilities
- Provide skilled nursing care in accordance with physician orders
- Document patient condition and care provided
- Follow patient care plans
- Communicate with the clinical team
Requirements
- Active NYS LPN License
- CPR Certification
- Infection Control Certificate
- Ability to work the assigned schedule
- Home care or PDN experience preferred
- Strong communication skills in English
- Passion for delivering exceptional patient care
Skills
- Bilingual in Spanish or Russian is a plus
