About the Role

We are seeking a caring and compassionate Hospice & Palliative MD leader to serve as Medical Director and join our growing hospice and palliative team. Patient care is our highest priority, focusing on helping patients find the comfort they deserve rather than billing codes. You will lead and provide the highest quality patient care within the hospice and palliative team.

Responsibilities

• Provide medical and administrative oversight of the medical component of Hospice and Palliative services.
• Offer clinical support and education to hospice and palliative multi-disciplinary teams.
• Collaborate with internal and external providers and referral sources to expand access to services.
• Work within an interdisciplinary team to consult with patients managing advanced symptoms, clarifying goals of care, addressing end-of-life issues, and transition planning.
• Conduct outreach to physicians and other providers to increase hospice and palliative referrals.
• Determine patient eligibility for hospice or palliative service per program acceptance criteria and Conditions of Participation.
• Certify and re-certify patient eligibility for the hospice or palliative program.
• Consult with primary physicians or specialists regarding patient's medical condition when not the primary attending.
• Assume responsibility for general medical needs of hospice or palliative patients if attending physician does not meet needs, serving as attending physician as needed.
• Serve as a liaison between the attending physician and hospice or palliative team, sharing patient's plan of care information.
• Evaluate patient's medical status in various settings based on a visit schedule.
• Provide administrative and medical leadership and guidance to the hospice and palliative team.
• Serve on the clinical leadership team.
• Participate in quality assessment and performance improvement initiatives.
• Serve on internal QAPI and PAC committees.
• Work with the interdisciplinary team to establish, implement, and review the Hospice plan of care.
• Attend Patient Care Conferences as requested.
• Develop, review, and evaluate medical standards, policies, and procedures for the hospice and/or palliative program.
• Develop and provide in-services for the Hospice Team based on identified learning needs.
• Provide ongoing community and professional education related to the medical community.
• Network with hospital staff to provide educational seminars on hospice.
• Assist with the design and development of a home-based palliative care program.
• Supervise the care of palliative care patients in the home-based program.
• Perform home visits and visits for inpatient hospice services for patient assessment and care.
• Work directly with patients, families, caregivers, and providers to create customized care plans for home-based palliative care.
• Assist the interdisciplinary team in developing and implementing a plan of care coordinated with the attending physician.
• Provide coverage and support to hospice staff for medical services in the absence of the attending physician or after hours.
• Render necessary medical management, in consultation with the attending physician, consistent with the plan of care and patient needs.
• Act as a resource to hospice staff, patients, family members, and attending physicians regarding pain and symptom management.
• Coordinate networking with community physicians to promote the palliative home-based program.
• Assist with the promotion of the palliative care program with the business development department.
• Understand and demonstrate agency's shared values, safety policies and procedures.

• Demonstrate understanding of team approach and communication.

• Adhere to the Agency’s policy and procedures regarding confidentiality and patient rights.
• Attend in-services/conferences required for position/program annually.
• Demonstrate flexibility by accepting other duties as assigned for which qualified.

Requirements

• Completed a Palliative Medicine Fellowship Program.
• BE/BC in Palliative Care.
• Possess a full range of clinical skills.
• Interest in working with an innovative hospice practice.
• Experience working in an interdisciplinary team delivering Palliative Care/Hospice Medicine.
• Unrestricted New York State Medical License.
• Federally Issued DEA license.
• Board certified in Internal Medicine or Family Practice.
• One to two years of medical physician experience including pain and symptom management experience.
• Proof of completed COVID-19 vaccine.
