About the Role
The Case Coordinator oversees the psychiatric and medical treatment regimens of residents, providing case management services after individualized needs assessments. This role involves maintaining case files and service plans, and may require using a personal vehicle for client transportation.
Responsibilities
- Obtain and maintain thorough knowledge of appropriate community resources.
- Serve as an advocate for residents and assist with securing needed services from other agencies.
- Initiate input from other staff and residents to develop service plan goals.
- Assign, oversee, and complete necessary assessment tools quarterly.
- Develop and monitor service plans with individual residents.
- Assist residents in developing personal and social skills, including personal hygiene, money management, conversation, and symptom management.
- Act as a liaison between the community residence and family members, providers, and other community contacts.
- Refer residents to appropriate social, rehabilitative, medical, mental health, and vocational services.
- Provide transportation for residents to other needed services with staff assistance.
- Act as a liaison to other direct care providers through mutual staffing.
- Maintain case records, including service plans, 90-day service plan reviews, and assessments.
- Obtain ongoing required medical records.
- Order needed medication prescriptions.
- Notify appropriate agencies of admissions and discharges.
- Attend weekly staff meetings and review client status with attending staff.
- Encourage residents to complete daily living skills.
- Effectively handle crisis situations.
- Assume roles and responsibilities of the Community Residence Manager during scheduled absences.
Requirements
- Must maintain a valid driver's license and acceptable driving record.
- Must maintain Standard First Aid and Cardiopulmonary Resuscitation certification.
