About the Role
Responsible for intake and referral processing. All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.
Responsibilities
- Receive and process referrals.
- Arrange DME, supplies, etc.
- If LVN, arranges medications.
- Inputs data, demographics, etc. into EMR
- Schedules hospice evaluations/admissions.
- Provides staff/IDT with patient admission information and documentation
- Obtain ROI for medical records and send out medical records requests
- Provide updates to MD’s and referral sources
- Educate families about hospice services and benefits
- Assist with transfers to Christopher House
- Upload medical records to patient charts
- Add admission nurses schedule to admission calendar and confirm admission appointments
- Notarize documents
- Respond to email requests from family members requesting information or admission.
- Email brochures, caregiver lists and facilities lists
- Verify Medicare benefits if working the weekend
- Daily referral follow ups
- Work rotating weekends and holidays (can be done remotely if you have all equipment required)
- Attend all Access/Admissions meetings and complete all education requirements
Requirements
- HS Diploma/GED required.
- One-year clerical experience in a medical environment required.
- Demonstrates competencies for populations served.
- Medical terminology knowledge.
- Ability to multitask and manage high call/referral volume
Skills
- Knowledge of Medicare/Medicaid hospice benefits
- Experience in hospice/home health admissions preferred.
