About the Role

Experienced Adults Social Worker needed for a Community Team in Croydon. This full-time agency role offers competitive pay and hybrid working, focusing on supporting adults to achieve independence and wellbeing.

Responsibilities

  • Manage a caseload of adults with varying levels of need, including complex cases.
  • Complete Care Act assessments, reviews, and reassessments.
  • Develop and review person-centred support plans.
  • Undertake safeguarding enquiries and manage risk effectively.
  • Complete Mental Capacity Assessments and apply the Mental Capacity Act.
  • Support hospital discharges and help prevent unnecessary admissions.
  • Work collaboratively with health services, housing providers, voluntary organisations, and other partner agencies.
  • Provide information, advice, and guidance to adults and their carers.
  • Maintain accurate, timely, and high-quality case records.
  • Contribute to service development and continuous improvement initiatives.
  • Support and mentor less experienced colleagues.

Requirements

  • A recognised Social Work qualification (Degree, DipSW, CQSW, or equivalent).
  • Current Social Work England (SWE) registration.
  • Experience completing assessments, reviews, and safeguarding interventions within Adult Social Care.
  • Good knowledge of the Care Act 2014, Mental Capacity Act, and related legislation.
  • Strong multi-agency partnership working skills.
  • Excellent communication, organisational, and problem-solving abilities.
  • A compassionate, proactive, and person-centred approach to practice.
  • Commitment to delivering high-quality outcomes for adults and their families.
