About the Role
As a Support Worker in the Mental Health Outreach Service, you will empower individuals with mental health challenges to achieve their goals. You will work within a diverse team to deliver person-centered care, promote independence, and improve the emotional wellbeing of service users.
Responsibilities
- Identify service user needs and develop care plans focused on life-enhancing outcomes
- Promote independence and emotional wellbeing for service users
- Develop respectful, supportive, and sensitive relationships with service users
- Assess, plan, implement, manage, and evaluate activities within a person-centered planning framework
- Help service users develop valuable life skills
- Collaborate with colleagues and teams involved in the care of assigned service users
- Produce written reports and statistical data to demonstrate service impact
Requirements
- Experience in a health and social care role
- Experience working with people who have severe and enduring mental health problems
- Experience working with families and carers
- Understanding of social inclusion, independence, empathy, and empowerment principles
- Ability to plan and deliver varied activities
- Enhanced DBS Check
