Responsibilities
- Teach skills that foster independence and community integration
- Follow care plans as written in the ISP
- Assess skill levels of persons served and provide appropriate assistance
- Engage persons served in activities
- Maintain accurate records of client progress, services, and personal financials
- Report changes in client condition to supervisor or manager
- Transport persons served to locations outside the home or program area
- Plan, shop for, and prepare nutritious meals
- Pass and monitor medications as delegated by the RN or supervisor
- Perform medical procedures as delegated by the RN or supervisor
- Administer personal care including ambulation and hygiene assistance
- Support the spiritual enrichment choices of persons served
- Perform housekeeping duties including cleaning, laundry, and dishwashing
- Maintain cleanliness of program vehicles and exterior areas
- Run errands for the program area or home
- Maintain a safe environment for persons served
- Support the mission and vision of the organization
- Participate in committees and meetings
- Adhere to all company policies and procedures
Requirements
- Ability to perform each essential duty satisfactorily
- High School Diploma or equivalent
- Equivalent combination of training and experience
Work Type
- Monday-Friday 8:30am-4:30pm
