Responsibilities

• Teach skills that foster independence and community integration
• Follow care plans as written in the ISP
• Assess skill levels of persons served and provide appropriate assistance
• Engage persons served in activities
• Maintain accurate records of client progress, services, and personal financials
• Report changes in client condition to supervisor or manager
• Transport persons served to locations outside the home or program area
• Plan, shop for, and prepare nutritious meals
• Pass and monitor medications as delegated by the RN or supervisor
• Perform medical procedures as delegated by the RN or supervisor
• Administer personal care including ambulation and hygiene assistance
• Support the spiritual enrichment choices of persons served
• Perform housekeeping duties including cleaning, laundry, and dishwashing
• Maintain cleanliness of program vehicles and exterior areas
• Run errands for the program area or home
• Maintain a safe environment for persons served
• Support the mission and vision of the organization
• Participate in committees and meetings
• Adhere to all company policies and procedures

Requirements

• Ability to perform each essential duty satisfactorily
• High School Diploma or equivalent
• Equivalent combination of training and experience

Work Type

• Monday-Friday 8:30am-4:30pm
