About the Role

Bristol Hospice is seeking compassionate Certified Home Health Aides (CHHA) with a heart for patients and families. As a CHHA, you will be a critical member of the Interdisciplinary Group Team, working under the supervision of a Registered Nurse to provide personal care and promote patient comfort. Bristol Hospice is a nationwide leader dedicated to providing compassionate, respectful, and dignified hospice services.

Responsibilities

  • Perform personal care needs and attend to patient requests promptly.
  • Report inability to perform tasks to the Case Manager immediately.
  • Utilize positive communication techniques, ensuring ethical care and confidentiality.
  • Ensure patient safety by using equipment properly.
  • Provide personal care, including baths, back rubs, oral hygiene, shampoos, and linen changes.
  • Assist with dressing and undressing patients.
  • Plan and prepare nutritious meals, including shopping.
  • Assist with feeding patients.
  • Take and record vital signs as ordered.
  • Provide proper skin care to prevent breakdown.
  • Assess and report patient condition changes to the Case Manager.
  • Be aware of the living situation and interpersonal dynamics of the patient's household.
  • Assist with ambulation and exercise as instructed.
  • Provide range of motion and transfers/positioning as directed.
  • Assist with self-administered medication reminders as allowed by state regulations.
  • Offer and assist with bedpan and urinal aid.
  • Aid with light laundry needs if necessary.
  • Perform range of motion and other simple procedures as ordered.
  • Provide respite for patient's family/caregiver when on-site.
  • Maintain a clean and orderly patient living area.
  • Adhere to organization's documentation, care procedures, and standards of conduct.
  • Participate in quality assessment performance improvement activities.
  • Respect patient and family/caregiver environment and personal needs.
  • Perform all other duties as assigned.

Requirements

  • Must be a Certified Nursing Assistant (CNA).
  • California residents must be a CNA in good standing and possess a current Home Health Aide (HHA) certification in good standing.
  • One (1) year of experience in community health/hospice or medical/surgical environments is preferred.
  • Must have completed a minimum of 75 hours of classroom and supervised practical training.
  • Must possess a valid driver's license with automobile insurance in accordance with organization/state requirements.
  • Must have a reliable and well-maintained automobile.
  • Willingness to submit to a criminal background check prior to hire and annually thereafter.
  • Must possess current CPR certification.

Skills

  • Ability to read and follow written instructions.
  • Understanding of hospice philosophy and comfort providing specialized care to the terminally ill.
  • Ability to work autonomously with little direct supervision.
  • Tact, patience, and good personal hygiene.
  • Effective communication skills.
  • Strong organizational skills.
  • Understanding of plan of care documentation.
  • Flexibility in work hours.
  • Ability to empathize with the needs of the ill, injured, frail, and impaired.
  • Comfortable addressing issues of death/dying.
  • Calm demeanor.
