About the Role

The Outreach Specialist coordinates outreach efforts for the Health Homes program, conducting home visits and outreach throughout Staten Island to identify new clients, re-engage lost clients, and work with community providers.

Responsibilities

• Coordinate with the Outreach Supervisor to identify clients for outreach.
• Conduct outreach services, including home visits, phone calls, and mailing letters, to enroll eligible clients.
• Complete progress notes and screening tools for clients.
• Drive an agency vehicle regularly to provide outreach services.
• Complete documentation in the program's database.
• Attend community events on some weekends.

Requirements

• High School Diploma or equivalent required.
• Experience working with the Homeless/HIV population preferred.
• Bilingual Spanish/English is a plus.
• Clean and valid driver's license.
• Ability to drive an agency vehicle.
• Exceptional communication skills, both oral and written.
