About the Role

Provide intensive, individually tailored and strength-based case planning services to high risk families referred through Erie County Department of Social Services during non-traditional hours, including weekends and evenings.

Responsibilities

  • Engage families in-home face-to-face and/or phone contact for a minimum of 1 hour per day to implement and support ongoing progress of the case plan.
  • Assess for safety and well-being of all family members through face-to-face and phone contact.
  • Address any immediate safety or elevated risk concerns identified during contact.
  • Ensure accurate, thorough, and timely completion and submission of progress notes detailing families' identified goals and outcomes related to safety, permanency, and well-being within 48 hours of contact.
  • Support the use of effective, culturally competent, trauma-informed techniques and interventions with parents/caretakers and children/youth to enhance family safety, permanency, and overall well-being.
  • Provide crisis support to families and communicate with agency staff as needed.
  • Occasionally provide transportation of children and families.
  • Actively participate in required meetings and training opportunities.
  • Perform other duties as assigned.

Requirements

  • Must have excellent communication and customer service skills.
  • Ability to multi-task and be detail-oriented.
  • Ability to work independently and be an enthusiastic team player.
  • Ability to work effectively with clients, staff, families, and community contacts.
  • Ability to work non-traditional hours, including weekends and evenings.
  • Valid New York State driver's license and adequate auto insurance.
