About the Role
The Care Manager I (CMI) is responsible for assessing the psychosocial needs of residents and coordinating support services for low-income older adults. This role involves planning, monitoring, and ensuring the provision of quality care to help aging adults and adults with disabilities maintain their quality of life.
Responsibilities
- Conduct comprehensive assessments and on-going re-assessments of residents' psychosocial, physical, mental health, environmental, and spiritual needs.
- Write comprehensive assessments.
- Develop and initiate client-centered Community Living Plans based on assessment information.
- Identify, arrange for, and monitor appropriate community services, utilizing knowledge of Medicare, Medi-Cal, and other entitlement programs.
- Establish and maintain care management relationships with clients and their significant others, offering respect, dignity, and support.
- Provide crisis intervention, advocacy, problem-solving, and therapeutic interventions.
- Meet with clients at least monthly, or more often as needed.
- Review and modify Community Living Plans on an ongoing basis.
- Document all case management activity and identified problems via progress notes within 24 hours.
- Maintain required paperwork and follow a clear, concise, and consistent charting system for continuity of care.
- Educate clients and significant others about resources and train them in case management when possible.
- Establish and maintain open and effective communication with community providers, including physicians and other health care and social service workers.
- Provide appropriate information on client care and program operations while maintaining confidentiality.
- Monitor the quantity and quality of services provided by other involved providers.
- Terminate clients when appropriate, in collaboration with the client, caregiver, and involved services.
- Participate in research studies and data collection as required.
- Participate in and promote Continuous Quality Improvement efforts.
- Attend and actively participate in team and program meetings, contributing to open communication.
- Utilize supervision appropriately, maintaining open communication and providing updates on caseload activity.
- Incorporate ethical standards of the National Association of Social Workers into all interactions.
- Understand and apply regulatory and procedural requirements of Community Living Services and Institute on Aging policies.
- Attend continuing education classes and/or in-service training to enhance knowledge and skills.
- Perform other reasonably related responsibilities as assigned.
Requirements
- One year of experience working with disabled adults and/or frail elderly.
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and the frail elderly.
- Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems, and teams of health professionals.
- Demonstrates case management skills and experience in the community health care delivery system.
- Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.
- Computer literacy required.
- Ability to lift up to 25 lbs.
- Ability to reach, bend, and walk.
- Five finger manipulation.
- Ability to speak and write English clearly.
