About the Role

Provide care and comfort to United Hospice's patients and families during a vulnerable moment, honoring their singular journey with compassionate care.

Responsibilities

• Provide direct spiritual care to patients and families as requested.
• Coordinate all aspects of patient care in accordance with professional standards.
• Contribute to patient and family counseling and education.
• Participate in ongoing interdisciplinary comprehensive assessments.
• Document spiritual care assessments, interventions, and outcomes according to UH standards.
• Contact community clergy on behalf of the patient/family.
• Assist in planning hospice-related events with a spiritual component.
• Participate in QAPI programs and hospice-sponsored in-service training.
• Participate in or conduct memorial services or funerals for patients, as requested.
• Assist in bereavement activities as appropriate.
• Maintain Point of Service documentation.
• Accept other responsibilities as assigned.
• Incorporate the organization’s commitment to customer service and continuous quality improvement into daily work habits.

Requirements

• Graduate of an accredited seminary or school of theology or advanced training in pastoral care/counseling.
• Knowledge of emotional effects of death, grief, and loss.
• Ability to be open, flexible, and ecumenical in ministry.
• Commitment to the hospice concept and its implementation.
• Proficient in Microsoft Office Suite.
• Two (2) units of CPE preferred.
• Ability to relate to a diverse population and workforce in a personable and effective manner.
• Must have access to a vehicle to visit patients in the community.
