About the Role

Provide housing case management services and support to adult individuals who are homeless or at risk of homelessness, with mental health, chronic health, substance use, and co-occurring issues. The goal is to help participants obtain and maintain safe, affordable housing and residential stability in the community.

Responsibilities

  • Demonstrate agency core values in day-to-day activities.
  • Assist individuals in locating safe and affordable housing in an area of their choice.
  • Promote and support individual movement through program phases, including assisting customers in developing goals and target activities leading to sustainable housing and successful graduation.
  • Purchase individual household furnishings for participant residences.
  • Assist with obtaining benefits, collecting income, diagnostics, and completing a rental stipend to determine monthly voucher amounts.
  • Maintain open communication with all treatment and service providers, families, landlords, and any other supports or networks the individual is working with in the community.
  • Assist in outreach and location of referred individuals.
  • Maintain accurate records, including progress notes in the Millennium Database, service planning, roster maintenance, rental stipends, income and homeless verifications, and other required documentation according to agency standards.
  • See individuals in person twice each month, preferably in their homes, with visit frequency dependent on client need.
  • Attend team and community-based team meetings.
  • Perform other duties as assigned.

Requirements

  • Must have own transportation.
  • Must possess a valid driver's license with a satisfactory driving record and possess a personal vehicle for job requirements.

Skills

  • Proficient in the use of computer software such as MS Word, Excel, Power Point, Desktop Publishing, Outlook, etc.
  • Knowledge of Community Health Systems.
  • "Qualifying Experience" means verifiable full or part-time experience working with populations with mental health, substance use, chronic illness, and/or co-occurring disorders.
