About the Role
The Village of Glendale Crossing has an exciting opportunity for a Nurse Practitioner to join their team, focusing on resident-centered social culture and delivering the highest quality of clinical care.
Responsibilities
- Support and embrace our resident-centered social culture and be proud to deliver the highest quality of clinical care to our residents.
- Act as an educator and clinician in developing a multi-disciplinary Village team.
- Provide a focus on clinical quality improvement outcomes, working with and supporting the inter-disciplinary teams in working towards targets in improvement.
- Provide support and guidance in completing assessments, ordering, interpreting, and communication diagnostic testing and procedures.
- Provide support for and lead resident centered palliative care services.
- Participate in the clinical review of Village quality initiatives.
- Foster exemplary relationships with residents, their families and the Village team to deliver holistic care to our residents.
- Provide specialized clinical services and expertise.
- Support clinical programs and quality of life initiatives.
- Continuously improve the quality-of-care services provided to our residents.
Requirements
- Hold a current registration as a Nurse Practitioner with the College of Nurses of Ontario
- Have experience completing comprehensive geriatric assessments
- Hold a current Heart Save Level of CPR
- Maintain a Current AIS Certification
- Experience leading, coaching and mentoring care professionals in a healthcare setting
- Display a caring and respectful attitude toward our residents, families, and team members
- Understand the physical, mental, and emotional demands of the role
- Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Long-Term Care Homes Act, 2007 and Health & Safety
