About the Role

The Village of Glendale Crossing has an exciting opportunity for a Nurse Practitioner to join their team, focusing on resident-centered social culture and delivering the highest quality of clinical care.

Responsibilities

• Support and embrace our resident-centered social culture and be proud to deliver the highest quality of clinical care to our residents.
• Act as an educator and clinician in developing a multi-disciplinary Village team.
• Provide a focus on clinical quality improvement outcomes, working with and supporting the inter-disciplinary teams in working towards targets in improvement.
• Provide support and guidance in completing assessments, ordering, interpreting, and communication diagnostic testing and procedures.
• Provide support for and lead resident centered palliative care services.
• Participate in the clinical review of Village quality initiatives.
• Foster exemplary relationships with residents, their families and the Village team to deliver holistic care to our residents.
• Provide specialized clinical services and expertise.
• Support clinical programs and quality of life initiatives.
• Continuously improve the quality-of-care services provided to our residents.

Requirements

• Hold a current registration as a Nurse Practitioner with the College of Nurses of Ontario
• Have experience completing comprehensive geriatric assessments
• Hold a current Heart Save Level of CPR
• Maintain a Current AIS Certification
• Experience leading, coaching and mentoring care professionals in a healthcare setting
• Display a caring and respectful attitude toward our residents, families, and team members
• Understand the physical, mental, and emotional demands of the role
• Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Long-Term Care Homes Act, 2007 and Health & Safety
