About the Role
The Case Manager serves as a primary liaison between clients, families, clinical teams, and referral partners within the Women's Program. This role provides structure, coordination, and empathetic support to individuals navigating substance use and mental health recovery.
Responsibilities
- Provide individualized case management throughout treatment and aftercare transition.
- Conduct weekly client meetings to assess progress and support adherence to the tier system.
- Coordinate client care to ensure alignment with clinical treatment plans.
- Communicate regularly with family members to provide updates and psychoeducation.
- Guide families in establishing and maintaining healthy boundaries.
- Maintain communication with clinical teams, therapists, and external referents.
- Collaborate on treatment planning and discharge strategies.
- Manage administrative tasks including bed tracking, contract timelines, and service extensions.
- Supervise and support Recovery Associates.
- Participate in weekly staff and leadership meetings.
Requirements
- Minimum of 2 years of experience in behavioral health, addiction treatment, or mental health.
- Strong written and verbal communication skills.
- Ability to manage multiple responsibilities while maintaining high standards of care.
- Comfort working in a fast-paced, team-oriented environment.
