About the Role

Northeast Family Services is a premier mental health provider with over 20 years of experience serving children, youth, and families. We are seeking a dedicated mental health professional to join our rapidly growing and well-regarded organization. This role involves serving as a liaison for the treatment team, client, families, and collateral service providers.

Responsibilities

  • Maintain assigned caseload and ensure proper implementation of in-home services per DCYF referrals.
  • Collaborate with DCYF to respond to referral requests, create treatment plans, participate in treatment meetings, and provide service-specific documentation.
  • Develop and collaborate on treatment plan formation with measurable goals in partnership with families and DCYF.
  • Adhere to all documentation requirements, including assessment and report writing.
  • Participate in reporting progress to the court system through appearances and written reports.
  • Provide parent education and supervised visitation to children, adults, and families in home and community settings.
  • Offer parent coaching and support, including effective parenting strategies, meeting healthcare and educational needs, understanding developmental milestones, meal preparation, and budgeting.
  • Enforce supervised visitation expectations as identified by DCYF to ensure child safety.
  • Interface with DCYF, the NH court system, schools, behavioral health providers, and other treatment team members.
  • Assess client risk on an ongoing basis and consult with clinical supervisor regarding safety concerns.
  • Transport clients, including children and adults, requiring car seat and booster seat installation.
  • Participate in on-call rotation to provide phone support to families.
  • Provide early evening availability for working hours, as most client visits are in the afternoon or early evenings.
  • Document all clinical contacts and documentation through the Electronic Health System.
  • Perform other duties as assigned.

Requirements

  • High school diploma with a minimum of 4 years of experience working with children, youth, and families in a home or community setting.
  • Associate's degree with a minimum of 2 years of experience working with children, youth, and families in a home or community setting (preferred).
  • Valid driver's license with reliable and consistent access to a vehicle.
  • Authorized to work in the U.S.
  • Access to high-speed, secure internet.
