About the Role
The Case Manager will support the residents' mental health and social care needs, ensuring access to primary care and navigation of health and social care systems. This model of care will ensure seamless and coordinated support for refugees settling in Toronto.
Responsibilities
- Provide on-site mental health and addictions support tailored to residents' needs, including developing personalized care and relapse prevention plans.
- Deliver settlement counseling services, including assessment, information, orientation, and shelter support.
- Facilitate internal/external referrals for services not provided by the program.
- Organize, plan, and facilitate group and individual sessions for skill-building, life skills training, and addressing diverse client needs.
- Develop and implement community outreach plans to promote programs and services.
- Collaborate with clients, programs, settlement services, and relevant parties to create individualized case management plans, including access to primary care, employment resources, and navigation through health, social care, and legal systems.
- Provide flexible support, including evenings and weekends, and assist residents with their migration journey and integration into the community.
- Offer transitional shelter support, develop safety and long-term housing plans, and create post-transitional support plans for ongoing housing and social care needs.
- Develop and codesign a ‘mentorship’ program for clients in the home.
- Conduct client assessment/individual service planning and develop therapeutic alliance with clients and their support systems.
- Build and maintain a trusting respectful and collaborative working relationship with client.
- Provide supportive counseling for a range of issues, skills teaching as well as psycho-education around illness, medications and symptoms.
- Develop, implement and monitor the service plan and conduct follow-ups with the clients as needed.
- Advocate on behalf of the client and connect them with community resources.
- Establish effective working relationships with staff of other agencies.
