About the Role

Provide individual/family counseling, treatment planning, assessments, and possible crisis intervention virtually or in-person for children, adolescents and their families to help prevent the progression of behavioral health needs.

Responsibilities

• Provide all clinical services within the context of the child and family environment, primarily in the child's home, school, or preferred community location.
• Offer telehealth services as appropriate and in alignment with state regulations and program procedures.
• Provide crisis management by stabilizing the child/youth in the home after the crisis has occurred and/or assist with goal setting.
• Provide clients and their families with goal-directed community services and solution-focused interventions to address behavioral health needs and achieve identified goals.
• Collaborate with the child and family to develop a diagnosis and treatment plan that meets medical necessity while honoring the child’s self-identified needs, goals, and ethnic, religious, and cultural identities.
• Establish a diagnosis using the DSM V and relevant assessment tools to create a medically necessary treatment plan with measurable goals, objectives, and supports.
• Work collaboratively with the child and family system of care, including Residential, Behavioral Health Clinic, Foster Care, Preventative services, Health Homes, Care Coordination, school districts, and primary care physicians.
• Complete progress notes via Electronic Clinical Record (ECR) within 24 hours of service provision.
• Adhere to all agency policies and procedures, including meeting Productivity Expectations.
• Adhere to Gateway's Code of Conduct and Code of Ethics, providing professional and meaningful interventions and interactions.
• Participate in clinical supervision, case consultation, staff meetings, and trainings as required.
• Perform other duties as assigned.

Requirements

• Must be license-eligible, hold a provisional license (LMHC-P or LMSW permit), or be fully licensed as an LMSW, LCSW, or LMHC.
• LMSW candidates should be eligible for or actively pursuing their “C” designation.
• LMHC candidates should be eligible for or actively pursuing their Diagnostic Privilege/Permit.
• Candidates may qualify under grandfathering provisions with appropriate exemption documentation.
• Valid New York State driver’s license.
• Adequate auto insurance.
• Reliable transportation to travel locally on a regular basis.

Skills

• Excellent documentation skills
• Ability to work independently and as an enthusiastic team player
• Ability to work nights and/or provide non-traditional hours, at least 2 days per week
• Knowledgeable in Evidence Based behavioral interventions and assessments
• Ability to work effectively with clients, staff, families, and community contacts
• Ability to travel to community locations to meet with clients
