About the Role
Seeking a compassionate and skilled Nurse Practitioner (NP) to provide high-quality, patient-centered care to individuals with serious, chronic, or life-limiting illnesses within a Home-Based Palliative Care Program. This role involves community medical practice integration and home visits to support patients.
Responsibilities
- Provide in-depth palliative care assessments and ongoing management for referred patients.
- Conduct home visits to assess clinical status, manage symptoms, and address patient and caregiver needs.
- Serve as a clinical resource within community medical practices for early identification and management of palliative care patients.
- Develop and implement individualized care plans in collaboration with physicians, social workers, and team members.
- Offer emotional and psychosocial support, including goals-of-care discussions and advance care planning.
- Coordinate care transitions with home health, hospice, hospital teams, and community resources.
- Participate in interdisciplinary team meetings and case conferences.
- Maintain accurate and timely clinical documentation.
- Diagnose and treat acute, episodic, or chronic illnesses.
- Prescribe necessary medications and treatments.
- Perform comprehensive physical examinations.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with collaborating physicians or specialists regarding patient care.
- Provide hospice care through evaluation, assessment, and management of health problems.
- Direct and develop the plan of care, coordinating with patients, families, and practitioners.
- Perform face-to-face assessments and evaluate hospice eligibility under physician supervision.
- Certify and re-certify patient eligibility for the Hospice Program.
- Provide clinical support to the hospice Multi-Disciplinary team.
- Consult with primary physicians or specialists regarding patient's medical condition.
- Assume responsibility for general medical needs of Hospice patients.
- Serve as a liaison between the attending physician and the Hospice Team.
- Evaluate patient's medical status in home, outpatient, and inpatient settings.
- Ensure adequate delivery of care 24/7 within the hospice provider team.
- Attend Patient Care Conferences.
- Provide ongoing community and professional education related to the medical community.
- Network with hospital staff to provide educational seminars on hospice.
- Assist with the design and development of a home-based palliative care program.
- Supervise the care of palliative care patients in the home-based program.
- Serve in a consultative manner to the patient’s attending physician.
Requirements
- Master's degree and graduate of a New York State-approved Nurse Practitioner program.
- Currently licensed and certified as a Nurse Practitioner by a national certifying organization acceptable to New York State in a related practice specialty area.
- Completion of the pharmacotherapeutic requirement enabling independent prescriptive privileges.
- Must have a National Provider Identifier (NPI) and Federal Drug Enforcement Administration (DEA) Registration Certificate.
- Previous experience in nursing or other medical fields.
- Ability to build rapport with patients.
- Ability to thrive in a fast-paced environment.
- Excellent written and verbal communication skills.
- Strong leadership qualities.
- Five years of experience, including pain and symptom management experience.
