About the Role
Assist individuals aged 60 and older by providing information, access to services, and support to maintain independent living. Responsibilities include screening, assessing, developing care plans, and facilitating referrals to appropriate programs.
Responsibilities
- Provide information and referrals to seniors, families, and service providers via phone, in-person, or community forums.
- Conduct client intakes to determine the need for case management services.
- Perform in-home assessments and develop, implement, and revise care plans.
- Advocate for clients with formal and informal support systems.
- Stay current with programs and resources pertaining to the elderly.
- Attend required meetings, trainings, and in-services.
- Utilize computer software for assessments, reporting, and correspondence.
- Maintain accurate and timely client case records in accordance with EISEP standards.
- Participate in case conferencing and consult with supervisors on complex cases.
- Intervene in crisis situations and refer to appropriate medical, mental health, or emergency services.
- Maintain a working knowledge of community resources.
- Supervise students, volunteers, aides, or other assigned individuals.
- Maintain confidentiality and handle sensitive information according to agency policies.
Requirements
- Bachelor’s degree.
- NYS Driver’s License meeting agency policy.
