About the Role
CBI Home Health supports people at home and in the community through services such as our Integrated Care Programs. These programs are designed to support people with complex care needs as they transition from hospital to home. Once home, our teams focus on helping our clients restore their health and function through a restorative care approach, so they can regain independence, enjoy their lives, and thrive in their communities. Integrated Care Programs are person-centered and team-based with a strong focus on communication, coordination, and doing what’s best for the individual.
Responsibilities
- Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
- Use a collaborative, interdisciplinary approach to care and services.
- Support a restorative approach to health, offering interventions, health coaching, and self-management support.
- Utilize digital platforms for care delivery, collaboration, documentation, and intervention.
- Support safe transitions from hospital to home through functional assessments, personalized interventions, and collaboration with care teams.
Requirements
- Registered member in good standing with the College of Occupational Therapists of Ontario.
- Ability to travel within the community.
- Valid driver’s license and insurance required.
- Embrace person- and family-centered care with passion and empathy.
- Exercise good judgment and critical thinking in client care decisions.
- Excellent communication skills.
- Thrive in both team settings and working independently.
- Value the role of a therapist in enabling clients to remain at home when it aligns with their personal goals.
