About the Role
As a PT in our Integrated Care Program, you’ll help patients rebuild strength and function after hospitalization, reducing readmissions and improving outcomes through hands-on, community-based care.
Responsibilities
- Help patients rebuild strength and function after hospitalization.

- Improve patient outcomes through hands-on, community-based care.
Requirements
- Registered in good standing with the College of Physiotherapists of Ontario.
- Ability to travel from home to home within the community.
- A valid driver’s license, vehicle and car insurance is required.
- A commitment to person and family-centered care.
- Passion, empathy, and critical thinking in client decision-making.
- Strong communication skills and the ability to collaborate within an interdisciplinary team while working independently.
- A deep appreciation for the role of a therapist in keeping clients at home when it aligns with their personal goals.
