About the Role
This position coordinates and delivers nursing care, implements facility policies, and performs admission assessments for the Rehabilitation unit. It ensures continuity of care by working with the Nurse Manager and Providers, maintaining high-quality care in accordance with all regulations.
Responsibilities
- Coordinate and deliver nursing care.
- Implement facility and nursing department policies and procedures.
- Perform admission assessments and documentation for new admits to the Rehabilitation unit.
- Complete assessments for new admissions within 48 hours or high-risk patients.
- Maintain contact with family and patient to ensure a smooth transition to the rehab process.
- Work closely with the Nurse Manager and Providers to maintain continuity throughout admission to rehab.
- Provide 24-hour management in accordance with federal, state, and local standards, guidelines, and regulations.
- Maintain the highest degree of quality care at all times.
Requirements
- Graduate of an accredited School of Nursing.
- Current licensure as a Registered Professional Nurse.
- Minimum of three to five (3-5) years of related experience in a healthcare facility.
- Minimum of two (2) years supervisory experience.
- Strong verbal, written, and computer skills required.
