Impact Health PC is seeking a Nurse Practitioner or Physician Assistant in New London, CT, to provide patient-centered care in skilled nursing facilities. Responsibilities

  • Deliver person-centered care using evidence-based practices
  • Collaborate with interdisciplinary facility teams
  • Conduct assessments, develop treatment plans, and manage follow-up care
  • Educate patients, families, and staff
  • Complete timely documentation (within 24 hours)
  • Ensure compliance with state regulations in SNF settings

Requirements

  • Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • Minimum 2 years of post-certification experience
  • Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
  • Proficient with EHRs and clinical technology
  • Strong communicator with a collaborative mindset

  • $115,000.00 - $200,000.00 Base+Commission/month
