About the Role
The Professional Practice Lead (PPL) leads and provides clinical support to a team of Client Services Supervisors (CSS), including Registered Practical Nurses (RPNs) and Registered Nurses (RNs). The PPL fosters collaboration with stakeholders, clients, and families to ensure client-centered care, adhering to nursing regulations and Infection Prevention and Control (IPAC) practices. This role works with organizational leadership and frontline teams to uphold and improve care standards.
Responsibilities
- Annually shadows each CSS within the community and conducts home visits.
- Creates a plan for continuous professional development and training.
- Collaborates with the manager to establish and maintain ongoing caseload realignment.
- Conducts monthly Peer support sessions with the client services supervisor team.
- Provides leadership in the development, evaluation, and improvement of clinical practice for the Client Service Supervisor (CSS) and Personal Support Worker (PSW) teams.
- Participates in researching, integrating, and promoting evidence-based clinical care models.
- Acts as a leader in change management initiatives and a champion for continuous improvement.
- Participates in the development of policies, procedures, processes, and tools to improve care delivery.
- Supports complex and difficult client clinical issues and complaints.
- Attends patient home visits and care conferences as required.
- Supports frontline staff with the development of care plans for complex clinical issues.
- Demonstrates commitment to the pursuit of results and service excellence.
- Reviews operational processes to identify opportunities for improvement and develops solutions.
- Participates in quality and process improvement activities.
- Participates in client satisfaction initiatives.
- Carries a reduced caseload of clients.
- Provides education and day-to-day support in the development of staff expertise.
- Supports on-boarding and orientation of new CSSs and PSWs.
- Supports continuous learning, professional development, and evidence-based care delivery through education, coaching, and mentorship.
- Supervises PSWs in a unionized environment.
- Supports the team to ensure department and/or client/family needs are met, including absence coverage.
- Develops professional working relationships with internal and external partners.
- Works respectfully, positively, and collaboratively within a team environment.
- Shares clinical and system knowledge, skills, experiences, and lessons learned.
- Actively participates in the Client Services Team, collaborating with and providing solutions/support to team members and leadership.
- Works with leadership to support coaching opportunities and performance issues.

- Assumes on-call responsibilities.
- Identifies and reports health and safety incidents and concerns in a timely manner.
- Documents incidents and escalates appropriately.
- Participates in health and safety processes and procedures.
- Cultivates a positive safety culture and encourages best practices for staff and client safety.
- Participates in all health and safety training initiatives.
- Takes proactive action against client incidents within scope of practice.
- Develops plans to identify, manage, and/or minimize client safety risks.
- Assesses the severity of adverse client safety/risk events and develops action plans.
- Collaborates with funders and follows additional processes as required.
- Calls emergency services (911) when a client is at immediate risk.
- Evaluates potential hazards and identifies clients at risk, taking preventative measures.
- Reports all safety events impacting clients, caregivers, and families in a timely and honest disclosure.
Requirements
- RN or RPN with current registration in the College of Nurses of Ontario.
- Two years of relevant working experience.
- Excellent assessment skills.
- Two years of experience supervising PSWs.
- Excellent interpersonal, problem-solving, and communication skills.
- Highly organized.
- Able to work independently while functioning as a member of a team.
- Demonstrated efforts to continuously upgrade skills.
- Demonstrated initiative regarding continuous improvement opportunities.

Salary/Compensations
- $65,000 to $76,500 per year
