About the Role

The Registered Nurse (RN) Care Coordinator is a vital member of the Community Prevention and Treatment Services (CPTS) Team, providing direct patient care to individuals facing infectious diseases and social determinants of health barriers. This role focuses on delivering gender-affirming care to diverse populations, including those with HIV/Hepatitis C, substance use disorder, and the LGBTQIA+ community, utilizing Patient Centered Medical Home principles to promote optimal health and wellness.

Responsibilities

  • Utilize evidence-based practice standards, PCMH guidelines, and chronic illness knowledge for HIV patient care management.
  • Collaborate with clinical teams to identify and coordinate care for the HIV patient population.
  • Coordinate care for patients receiving HIV and gender-affirming care, including referrals and medication education.
  • Participate in State and Federal Ryan White Program HIV trainings, conferences, and meetings.
  • Engage in HIV-related educational events with patients, including the quarterly Client Advisory Board (CAB).
  • Track HIV metrics and participate in quality improvement projects related to HIV and gender-affirming care.
  • Facilitate referrals to behavioral health, substance abuse, specialty care, and community resources to address SDOH.
  • Support patients living with HIV with medication education, community linkage, and counseling for viral suppression.
  • Participate in HIV clinical meetings across the WYH network.
  • Assist patients with linkage to counseling across the HIV care continuum, including STI testing, PrEP, and PEP.
  • Collaborate with medical case managers to ensure timely connection to care for all HIV patients.
  • Serve as a point of contact for patients needing assistance with clinical HIV needs and triage accordingly.
  • Demonstrate excellence in customer service and patient engagement.
  • Communicate HIPAA compliance, corporate compliance, and client confidentiality.
  • Ensure compliance with local, state, and federal regulations (FQHC), accreditation standards (Joint Commission, NCQA-PCMH), and National Patient Safety Goals.
  • Complete other duties as assigned.

Requirements

  • Associates degree/graduate of a registered approval program for Registered Professional Nurses with current NYS registration.
  • Two (2) years of experience in a healthcare setting.
  • Demonstrated excellent customer service, communication, and interpersonal skills.
  • Proficiency with computer use.
